This list is not comprehensive because each insurance company and personal plan differs from patient to patient. For full benefits information contact the customer service number on the back of your insurance card to verify benefits level and potential responsibility.

Invoices & Bills

Deductibles, Copays, and Coinsurance

Please pay your copay amount each visit if you know what it is. If you have a deductible or coinsurance, the insurance company will figure out the amount you owe based on your plan. You will be sent a statement called an Explanation of Benefits (EOB) from your insurance company with the amounts you owe. Please submit payment accordingly. If that payment is not made, then we will generate a bill based upon federal and state laws and send you a bill with the amount due.

Payment Options

If your deductible is high, then we can make a graduated payment plan that fits your budget. If you are not able to pay the bill, please call us and we will attempt to work something out. Remember, this is a low volume clinic and the services are rendered in advance of payment. We cannot operate as a facility if you do not pay for the services that where rendered to you.
